What is Membership medicine or Concierge medicine?

Membership medicine is a form of medical practice that is designed to focus on wellness and disease prevention .Since this new model will allow me to see fewer patients I can spend more time with each of you and concentrate on your wellness goals.
You will have direct contact with me via email and cell phone which helps to restore our physician-patient relationship and can often decrease the need for office visits and possibly prevent hospitalization.

How is membership medicine different from a traditional medical practice?

In a traditional practice a physician carries a panel of well over thousands of patients. This translates in to long wait times and short office visits. Both time and reimbursement pressures affect the physician-patient relationship.

I am transitioning to a membership program which will eliminate these external pressures and I will have the time needed to focus on your health and wellbeing.

Do I still need health insurance?

Yes, your insurance plan will cover necessary interventions including labs, x-ray, hospital and specialist fees. Your private / concierge medicine is to improve your health by providing extra ordinary and connected primary care.

What benefits does the membership program offer?

The membership fee will cover conveniences and amenities that your health plan will not cover.

What is the fee structure?

Membership fees are billed directly to you via a monthly or annual fee which may be paid by personal check or credit card. Our founding members will join at a special lower rate. Please call our office at 650-568-5800 for more information.

Will my insurance reimburse my monthly or annual concierge fees?

This fee may not be submitted to Medicare or your insurance provider. Check with your CPA or tax advisor about using your Health Saving Account (HSA) or FSA funds.

Do you bill my insurance or Medicare?

I will only bill Medicare and your PPO insurance for sick visits and services in our office that are covered by Medicare and your PPO plan.

What if I need to see a specialist?

I will assist you in choosing the best specialist for your needed care. I will make appropriate arrangements so your follow up will be seamless.

How do I reach Dr. Ebadi?

During office hours you can call our office directly. Our staff will convey the message and I will personally call you back. After hours I can be reached directly by cell phone. You can also communicate via e-mail for non-urgent questions and I will respond within 24 hours.

Who will benefit from being a member?

Since this practice is focused on wellness and disease prevention everyone will benefit from joining this style of practice. It is an investment in your health and well being. You will enjoy first class healthcare and a range of conveniences to fit your health, work and life style needs.

Will I still benefit from being a member even if I don’t expect to need frequent medical attention?

Absolutely. Many people believe they should only treat illness when it happens instead of identifying and preventing disease before it starts. In my practice I not only see you when you are ill, but my focus is your long-term health and wellness. I will work with you to achieve your optimal health goals.

What happens if Dr. Ebadi is on vacation?

Even if I am out of town I will often be reachable by phone, but if I am not available, I will have another quality physician available to assist you for any urgent matters.

What if I want to end my membership?

You are welcome to cancel your membership at any time as long as you give us 30-day notice. We will refund memberships paid for the full year at a prorated basis.
